Patient Disclosure

Prior to surgery, one of our pre-admission nurses will be calling you to go over your instructions prior to surgery. These calls typically take place one week prior to your surgery and it will be information necessary to prepare you for your surgery date and normally take about 10 minutes. The nurses will do a brief questionnaire with you that will assess your physical condition and will ask for a current list of medications that you are presently taking.

You will be expected to pay any co-payment or deductible on the day of surgery. The facility accepts Visa, Mastercard, Amex , Care Credit or your personal check.

Please take a moment to review the forms on the right applicable to your surgery. You will be asked upon check in if you reviewed your patient rights and responsibilities, advance directives and physician owner disclosure.

It is important to have a responsible adult who will be able to stay with you during surgery for the first 24 hours and be able to drive you home from the facility.

Following surgery, you will be given a patient satisfaction questionnaire. We want to hear from you, so please take a moment and let us know if we met your expectations.
